As we continue to grow, we are seeking a dynamic and experienced Residential Care Officer to join our team. Position Overview: We are excited to announce an opening for a Residential Care Officer who will work at Cocoon SDA Care, a leading provider of specialist disability accommodation (SDA) and personalized care services under the National Disability Insurance Scheme (NDIS). This role is pivotal in providing personalized care, assisting with daily activities, and promoting independence for residents with disabilities. This role involves fostering a safe, inclusive environment in well-designed homes and collaborating on tailored support plans. Tasks & responsibilities · Care and supervision of disabled persons · Assess the participant's needs and assist them in planning and accessing health or welfare supports · Guide support workers about the welfare needs of the participants · Liaise with allied health professionals for matters relating to the health and welfare of the participants · Participating as required in training and development courses relevant to the service · Participating in and actively contributing to staff development reviews · Responsible for care and supervision of disabled (participants) · Conduct interviews and assessments to evaluate clients' physical and emotional well-being and understand their specific challenges · Collaborate with clients' families and healthcare professionals to develop and/or implement tailored care plans · Liaise with allied health professionals for matters relating to the health and welfare of the participants · monitoring and reporting on the progress of clients · Review medical records and refer clients to suitable residential facilities that meet their needs · supporting families and providing education and care for disabled persons. · Monitor and document changes in clients' physical and mental health within a residential setting · Guide support workers about the welfare needs of the participants · Foster connections with welfare agencies, community organizations, and public or private sectors to enhance access to community services. · Referring clients to agencies that can provide additional help Qualifications and Experience · Diploma or higher in the relevant field. · Should have one (1) year of relevant experience.
